Booking Form

A total of ($75 x Total Event Hours) will be paid to Lost and Found Healing/The Lost and Found Studio, by the due dates listed below under payment terms.
 

Cancellation Terms:

Cancellation more than (5) business days of the proposed event date will allow for (1) rebooking date using the deposit already provided, or loss of deposit should no new date be selected at time of cancellation.

Less than (5) business days of the proposed event date will result in a cancellation fee of 20% of the remaining booking total.

Code of conduct:

- The Lost and Found Studio is a safe community space, its occupants and the space, will be treated with care and compassion.
- While basic cleaning is included, I acknowledge I am responsible for leaving the space reasonably clean. Should the space be left in an unreasonable state, I understand that I may be obligated to pay a cleaning fee.

- The Lost and Found Studio is a cigarette smoke free environment.
- Alcohol is not permitted at any events without proper licensing.
- All  City of Toronto noise by-laws will be adhered to.
- The Studio is a shoe-free space.


I agree to agree to the terms stated in the above rental agreement and will uphold the code of conduct and agree to take responsibility/liability for any of my clients/vendors/staff or attendees who do not uphold the code of conduct.
